Client Referral Form. If you submit a client referral on our website using our Referral Form, we ask you for your first and last name, organization, phone number, email address, information about the referred client (such as name, contact information, pregnancy information), program preferences, and other information you choose to provide.
Communications. If you contact us directly, we may receive additional information about you. For example, when you contact us using our “Contact Us” form, we will receive your name, email address, the contents of a message you may send to us, and other information you choose to provide. If you subscribe to our newsletter, then we will collect certain information from you, such as your first and last name and email address.
Careers. If you decide that you wish to apply for a job with us, you may submit your contact information and your resume via e-mail. We will collect the information you choose to provide on your resume, such as your education and employment experience.
Birth Doula Services Program . If you would like to work with our Birth Doula Services Program, we ask you for your first and last name, email address, phone number, website, the areas you serve, a description of your services, why you are interested in working with us as a doula, as well as information about your preferences and other information you choose to provide.
Volunteers. If you would like to volunteer with us, we ask you for your first and last name, e-mail address, information about the frequency and times you would like to volunteer, how you heard about us, the tasks that interest you, your skills and goals, and any other information you wish to provide. We may also collect information about your volunteer hours, such as the dates and times you volunteered and other information you choose to provide.
Lactation Lounges. Realizing that families still need lactation support during the current health crisis, we are offering Virtual Lactation Lounges via Zoom video conferencing. Parents and caregivers can drop in during any stage of their lactating journey to ask questions and discuss challenges surrounding feeding. We will collect any information you choose to provide during these video conferencing sessions.
Donations. If you make a donation, your payment-related information, such as credit card or other financial information, is collected by our third-party payment processor on our behalf.
- Information We Collect When You Use Our Services
Device Information. We receive information about the device and software you use to access our services, including IP address (which may provide us with your general location information), web browser type, operating system version, phone manufacturer, and device identifiers.
Usage Information. To help us understand how you use our Services and to help us improve them, we automatically receive information about your interactions with our Services, like the pages or other content you view, the searches you conduct, donations you make, and the dates and times of your visits.
Information from Cookies and Similar Technologies. We and our third-party partners collect information using cookies, pixel tags, or similar technologies. Our third-party partners, such as analytics partners, may use these technologies to collect information about your online activities over time and across different services.
